Notice of Infringement – Claim
- A physical or electronic signature from the copyright owner (or an authorized representative)

- Details of the infringing material to be removed, along with information to help locate it
- Contact information for the complaining party including name, address, email, phone number, and fax number
- A statement by the complaining party that they believe in good faith that the material’s usage is unauthorized
- A statement that the provided notification is accurate and the complaining party is authorized by the copyright owner
Section 17 USC §512(f) imposes civil damage penalties, including expenses and legal fees, on anyone misrepresenting information in a notification of infringement under Section 17 USC §512(c)(3).

Counter Notification – Restoration of Material

- A description of the removed material and its original location
- A statement under penalty of perjury that you believe the material was taken down due to a mistake
- Your name, address, phone number, and consent to the jurisdiction for any legal matters

Repeat Infringer Policy
